Form + Function wasn’t built to be like other dental practices. Dr. Kevin designed it from scratch because the dental experience patients deserved didn’t exist. Every detail reflects his artistic vision, intentional design, and deep desire to create a patient experience that feels different.

Your first visit begins with open arms, welcoming smiles, and a seat in our inviting consultation room for a genuine conversation between you and Dr. Kevin. Before you even see a dental instrument or dental chair, we want to understand who you are, what you hope to achieve, and what concerns you have.

Do you accept new patients?
Yes, and we’re glad you found us! We welcome new patients, do our best to fit in with your schedule, and offer same-day appointments for dental emergencies.
Understanding Insurance: In-Network vs. Out-of-Network
We accept most PPO dental insurance plans and are in-network with a select few. Even if you’re out-of-network, you can still use your benefits here. We’ll handle the billing on your behalf to make the process as seamless as possible for you. All our pricing is transparent, so you’ll always know what your insurance covers so there are never any surprises.
Do you offer financing?
Yes. We offer convenient financing options for patients without insurance or for treatments not fully covered. Ask our team for details.

A genuine conversation with Dr. Kevin, a comprehensive oral health assessment using the latest techniques and technology, an oral cancer screening, a comfortable cleaning with our air polishing system, and a comprehensive pathogen test to identify bacteria, fungus, or virus in the mouth.
